I have ensured that my child understands that it is important to follow any rules/instructions given by the leaders in charge of the activity.

I understand that, although due care will be taken, neither the activity leaders nor ChristChurch Tilehurst will be liable for any injury or loss of personal property.

I have, or will have by the 13th May, paid £23 into ChristChurch Tilehurst's account to cover the cost of travel and the event ticket.
